Georgia Medicaid CCSP

Community Care Services Program (CCSP) is a Medicaid waiver program that provides home and community-based waiver service programs that provide social, health, and support services to eligible consumers as an alternative to placement in a nursing home. Service Options Using Resources in a Community Environment (SOURCE) is a State Plan enhanced primary care case management program that serves frail elderly and disabled beneficiaries personal support services, assisted living services, extended home health, home-delivered meals, etc. to qualified individuals.
